Subj: Change in Application Procedure for AAPD Waiver
Effective July 1, 2001, the application procedure for Alternatives for Adults with Physical Disabilities (AAPD Waiver) will change to follow the ElderChoices application procedure. Currently, individuals inquiring about the program are referred to the Division of Aging and Adult Services toll free number for an application packet. After receiving the application packet, they are instructed to make the application at their resident county office.
Effective July 1, 2001, individuals requesting AAPD Waiver services will apply at the DHS county office in their county of residence. Immediately upon receiving the application for AAPD Waiver, the caseworker will refer the individual to the DAAS Waiver Counselor via the DHS-3330. The Waiver Counselor will make a home visit to each applicant referred to develop a Plan of Care and to determine medical necessity.
Effective July 2, 2001, all callers to the DAAS toll free number will be directed to their local county offices to apply.
