Subj: SSN Enumeration Procedures When A Request For An SSN For A Child Was Made At A Hospital
Some hospitals make application an SSN for babies prior to their release. The purpose of this directive is to explain the procedure to use to apply for an SSN for a child when the client indicates that a request for an SSN was made at the hospital when the child was born but the number has not yet been received. This procedure will apply to all programs, (AFDC, Medicaid and Food Stamps).
An SS-5 and an EMS-12 must be completed by the county office for submission to SSA. A Pseudo SSN will be assigned to the child and entered on the SS-5 and EMS-12 as in normal procedures. The Service Representative will check the block on the SS-5 that states it is not known if an SSN has been applied for prior to this application. All other usual procedures for submitting an SS-5 to SSA will be followed.
