Subj:  Food Stamp Program Changes

[DCO-205] – The DCO-205, Referral Form & Participation Record, is used in counties where the E&T Program is operating to refer food stamp recipients to the E&T Program.

The DCO-205 is available on DHS GOLD.

[DCO-260] – In all counties, the DCO-260, Notification of Work Registration, must be provided at initial application approval and recertification to each household with a member or members subject to the work registration requirements. The DCO-260 must also be provided to a household when a reported change or quarterly report is processed and a household member becomes subject to the work registration requirements.

In counties where the E&T Program is operating, the DCO-260 must be used to notify the household when a household member is referred to the E&T Program.

The DCO-260 is available through the warehouse and on DHS GOLD.

[DHS-0187] – The DHS-0187, Billing and Routing Sheet, must be used when the E&T Program requests reimbursement for travel expenses incurred by an E&T Program participant. (The E&T Program worker should provide documentation of the travel expense.)

The attached copy of the DHS-0187 may be copied and used as needed in counties where the E&T Program is operating.
