AAPD

AAPD is a Medicaid Waiver program that provides home and community-based services to adults with physical disabilities.

What services are available?

AAPD offers the following services:

  • Attendant Care: Assistance in accomplishing tasks of daily living based on need and approved by a physician. Based on need, a client may receive up to 8 hours a day, 7 days a week of attendant care. There are two types of attendant care. In consumer-directed attendant care, clients recruit, hire, supervise and approve payment of attendants. In agency attendant care, clients choose an agency to provide an attendant for them. Although the attendant may be a family member, it may NOT be a spouse or other person legally responsible for the client.
  • Environmental Accessibility Adaptations/Adaptive Equipment: Adaptive equipment and modifications to the living environment that increase independence or accessibility.

  • Case Management and Counseling Support: Counseling Support Managers provide orientation to the concept of consumer-direction and training on how to recruit, interview, hire, evaluate, supervise, manage or dismiss attendants. Counseling Support Managers provide support services to all AAPD waiver clients, monitoring their needs and assisting the client in continuing to live as independently as possible.

Who can apply?

Anyone who:

  • Has been declared physically disabled through SSI/SSA or Medical Review Team

  • Has income of no more than 300% of SSI and countable resources of less than $2,000 (individual) or $3,000 (couple)

  • Is between 21 and 64 years old

  • Meets eligibility for Intermediate Level Nursing Home Placement
    Note:
    To be eligible for Intermediate Level Nursing Home Placement, an individual must require extensive assistance with 1 ADL or limited assistance with 2 ADLs (activities of daily living, ADLs, are considered transferring/locomotion, eating or toileting), or have a diagnosis of dementia, or have a medical condition that requires daily monitoring by a medical professional. Individuals that require skilled care cannot be served.

How do I apply?

Contact your local DHS County Office to apply for Alternatives. An Alternatives RN/Counselor will arrange to visit you at home to complete an assessment, which must be approved by your physician.

What is the role of the AAPD RN/Counselor?

The AAPD RN/Counselor will:

  • Complete an evaluation that will be the basis for determining eligibility;

  • Develop a plan of care based on the needs and wishes of the client;

  • Assist in enrolling providers of choice;

  • Complete an annual reassessment of needs; and

  • Monitor in-home service costs to assure that costs do not exceed the cost of nursing home placement.
