The renewal period for the following individuals and facilities closed as of April 1, 2011:
- Pharmacy Technicians
- Charitable Clinic Pharmacy Technicians
- Nursing Home Consultants
- Hospital Pharmacies
- Institutional Pharmacies
- Wholesale Distributors of Legend and Controlled Substances
- Wholesale Distributors of Home Medical Equipment, Legend Devices, and Medical Gases
- Wholesale Distributors of List I Chemicals
- State Supported/Charity Clinics
Renewal information for these individuals and facilities was sent out on October 18, 2010 to the mailing address that we have on file. If you have not renewed your license by April 1, 2011, your license is void.
Continuing to practice or operate in the state of Arkansas without proper licensure is illegal.
Pharmacy Technicians who wish to begin the process of reinstating their licenses can contact Nancy Sweet for a pharmacy technician reinstatement application at 501-682-0194 or at nancy.sweet@arkansas.gov. Please include your name, address, and license number in your request.
